Juvéderm Ultra 4 is a sterile, pyrogen-free physiological solution of cross-linked hyaluronic acid of non-animal origin, presented in a graduated, pre-filled, disposable syringe. Each 1 mL contains hyaluronic acid gel 24 mg and lidocaine hydrochloride 3 mg in phosphate buffer at pH 7.2, the lidocaine being included to reduce the patient's pain during treatment.

Juvéderm Ultra 4 is an injectable medical device for administration exclusively by licensed, appropriately trained healthcare practitioners in a controlled clinical setting. It is not supplied for retail sale and never for self-administration. Deeper placement carries a correspondingly higher requirement for anatomical knowledge, particularly around the vasculature of the midface, and the injection plane, volume and technique are the practitioner's decision. It must not be injected in the periorbital area, meaning the eyelids, under-eye area and crow's feet, or in the glabellar region. It must not be injected into blood vessels, since intravascular injection may lead to embolisation, occlusion of the vessels, ischaemia or infarction, and it must not be overcorrected. Ultra 4 must not be exchanged for Ultra 2 or Ultra 3, which are placed at different depths and supplied with different needles.

For adult patients assessed as appropriate by the treating practitioner. Selection, injected volume, plane and technique follow clinical assessment by the practitioner. The product must not be used in patients suffering from untreated epilepsy, patients who tend to develop hypertrophic scarring, patients with known hypersensitivity to hyaluronic acid or to gram-positive bacterial proteins, patients with known hypersensitivity to lidocaine or to amide-type local anaesthetics, patients suffering from porphyria, women who are pregnant or breastfeeding, or children. It must not be used in areas presenting cutaneous inflammatory or infectious processes such as acne or herpes.
